VA/DoD Diabetes Clinical Practice Guideline Toolkit Update Meeting

OBJECTIVES

1. Become familiar with the key recommendations of the VA/DoD Clinical Practice Guideline for Diabetes
2.  Identify the gaps in knowledge for a provider managing the care of a patient with DM in a medical home care setting.
3. Identify barriers for implementation of the CPG
4. Define the types of tools that would assist a provider in the proper screening assessment and treatment of patient with DM.
5. Review and select key provider education tools for development or adaptation
6. Ensure that tools selected will decrease the burden of CPG implementation on providers and patients within the VA and DoD medical treatment facilities.
7. Identify metrics for tracking outcomes related to the implementation of the DM guideline
